[Content bootstrap] nodes with combine or overlay directive should not be created implicitly

This is similar to REPO-988. In this case however nodes are implicitly created during the application of enhanced delta xml files: when the processor encounters a node with a combine or overlay directive, a new node is added when the node does not already exist. The correct behaviour should be for the application of the delta to fail at this point.
