    Description

      Please see the attached 'create-content-step-1.png' image.

      Acceptance Criteria

      1) Set focus on the document name field

      2) Make sure the document type field label is displayed (iow: fix CHANNELMGR-1944 )

      3) No longer make the Document location field disabled, so that the field's browse button is part of the tab order

      Reason for ticket: users think they have to do something with the folder path displayed to them because of its design; this is an incorrect interpretation and we need to make it clearer to users that this path doesn't actually need changed as part of the create-content flow
