Make sure index names are unique in case there are multiple unique index columns

Note that this issue does NOT manifest itself because currently, the

final String ... uniqueIndexes

in DbLockManager#createTableIfNeeded always contains one value. If however this is every reused to create another table, we might run into problems because the index name is the same all the time
