Improve reading and writing of RichText fields

    Description

      When reading or writing a richtext field (so whenever any character is changed while editing the document) all subnodes are removed and rebuilt based on the uuid in it's html source. This is obviously very inefficient; a sub-node should only be changed, removed or added when it's reference is changed in the html text.
