The date field in easy-froms is not reflected in channel manager or site

1. Log-in to CMS
2. Navigate to Hippo Go Green Green >> Common >> Contact Us form and click edit
3. Add a drop down field for sex with values (male & female)
4. Add a Date field for DOB
5. Save & close the document and publish it.
6. Now click on View and select English website

Expected - Both the newly added fields Sex and DOB should be shown in channel manager
Actual - Only one field is shown, DOB is not shown (refer screenshots)
