Issues with table headers in rich text fields

When adding a table in a rich text field, there is an option to include a table header. There are several issues with this, some might be product issues (CMS7-8293), others project issues (HIPCON-1314).

After adding the table, the table header cells is not visually different from regular table cells. Only when changing to html source mode can one see that there is actually a table header.

After saving and closing the document, the table header is not visible in the non-editing view of the document.

In the web site the table header is not visible either.
