Add cache busting in ExtJS for the index.html files of AngularJS apps

A browser caches the index.html files of the AngularJS apps in the channel manager (i.e. the 'edit menu' window, the 'pages' window, and the 'page settings' window). When a newer version of these index.html files is available in the channel manager jar files, a browser must fetch the newer version.
