Several UI and interaction issues after merging the SCXML DocumentWorkflow branch

I noticed the following UI and interactions issues:

• After editing and changing a document, the preview no longer shows the changes. The same is true when viewing a revision.

• When an author requests a (de)publication and another editor accepts or rejects this request, the menu for the author is not refresh/notified. The author actually has to close and reopen de document to get an refreshed menu. This also affects other menu items like the 'Edit', which stays disabled until reopening the document.

• When an editor rejects a publication request, its menu item isn't refreshed, e.g. accept/request remain active and (for instance) edit disabled. This is not the case when accepting the request (probably because that triggers the actual publication and thus changes on the variants).

• Save/Save and Close menu items requires twice clicking to activate. Possibly other menu items too?

• The order of the menu items in the publication menu is different than 7.8. The order should be the same as in 7.8.

More problems related to this SCXML/DocumentWorkflow merge can be added to this issue later.
