Steps to reproduce
1) Login in the CMS (Tested with archetype and Go Green Enterprise admin/admin)
2) Create a document and publish it
3) Go to Document/Revision History
Expected:
Information on Name, Date / time, User and State
Actual: Name column is missing. Only Date / time, User and State are shown
