Improvements to apply patch dialog when node already exists

Consider scenario where 2 nodes had to be added in patch.
1 is new (Mathijs Top) and 1 already exists (Mathijs).

The feedback we got is (I also added a screenshot):

Some patch operations failed
Action : insert aboutmenu/Mathijs@5: success = true
Action : insert aboutmenu/Mathijs Top@7: success = false

This is very descriptive. You know it didn't work, but not why it didn't work. The log file is a lot better:
Caused by: javax.jcr.ItemExistsException: This node already exists: /hst:hst/hst:configurations/hippogogreen/hst:sitemenus/aboutmenu/Mathijs
