Message in activity stream when user logs in for the first time

The message is like:

One minute ago, editor added "editor"

When the message is clicked, a document is opened, named "editor", without apparent contents. The path is: "/collections/editor"
There are options to "Publish documents" or "Take offline". It is unclear if using these has any effect.
