Adding translation to a document fails sometimes

• select any document that needs to be translation
• Click on Languages and select the language (with a + sign)
• In the "Translate Documents" dialog click on the "add translation" [red] label.
• Type the name for the document and press enter
• Click on OK

"Action failed" Message is displayed.
