Error while creating custom Document Type

We are trying to create a new Document Type using the web interface (/cms) and for some reason getting the an error: "Error while executing action: No template defined for add to folder" (see attached screen shot). This error happens after you click the "Finish" button.

We have stood up several other environments and have never seen this error before. Not sure what would have happened in this installation?
