URL Rewriter: undo the numeric prefixing of the document type names

In HIPPLUG-382, to have a specific ordering in the new document dialog, the document type names were prefixed with "1. ", "2. " and "3. ".

Please undo this since the document type names also show up on other locations, like the document type listing (see attached).

Also document this in 1.05.02 and .05.03 release notes and release it.
