Taxonomy plugin (v1.02.00): first time adding taxonomy items

adding taxonomy items in different level will only display the last added item

Path to reproduce:
1. edit the project document, add category(1), add category > sub-category(2), and add category > sub-category > catA(3)
2. click ok, the taxonomy box will only display catA
