URL Rewriter: bump urlrewritefilter version to 4.0.4-h2

Version 4.0.4-h2 of the urlrewritefilter on github/onehippo has been released, with an upgrade of httpclient library. See https://github.com/onehippo/urlrewritefilter.

Let's use that new version in the Hippo 11.2 based URL Rewriter plugin and release as 2.2.0.

Secondly, start using the onehippo/urlrewritefilter on the 10.2 based URL Rewriter and release as 1.06.03.
