Review Workflow Plugin - Allow multiple review requests

Limitations to address with this implementation:

1. The plugin allows only one review request at a time. 
2. The message on the yellow ribbon can include details about the user/group which is assigned to
3. The plugin is not extensible because all the wf actions are created in the constructor
4. Bugs and refactors
