Content blocks plugin doesn't delete provider fields in document types

There are quite some more issues with the content blocks feature:

• when there are document types with provider fields, adding a new provider breaks the document type editing functionality.
• when a new provider is added while the UI contained changes to the document types, these changes are lost.
• UI suggests support for deleting a provider, while that's not the case (completely)
• UI is hard to understand
