actions view does not update when doing an action

When you click for example on 'request publication', nothing seems to happen. Under water, everything is right. Again clicking on the document shows the changed actions view. This should be visible directly after the action.

Also after 'request publication', should this option not be removed? You can click 'request publication' resulting in a workflow exception:
