Dialog: no visual feedback that the user clicked a button

• add a new document
• in the dialog, enter a name and select a doctype
• click "ok"

At this moment, the user should get some visual feedback that the document is actually being created.
Either by:

• Setting the mousepointer to an hourglass
• Displaying the AJAXLoader as used when updating all content (see screenshot 2)
