Merge workflow: merge document actions, requests

Merge and rename the boxes for "cms", "Request", "Publishing", "Item" as follows:

• cms (with document icon) (=name of the document)
• Edit
• View (shows 'versioning' preview)
• Take offline (if someone is allowed to do this, then do it, otherwise, render as "Take offline (request)")
• Push live (if someone is allowed to do this, then do it, otherwise, render as "Push live (request)")
• Delete (if someone is allowed to do this, then do it, otherwise, render as "Delete (request)")
